Treating frown lines: what really helps – and what doesn’t

Frown lines – medically called glabellar lines and often nicknamed the “11s” – are the vertical lines between the eyebrows. They form when small forehead muscles contract again and again, and they can make a face look stricter, more worried or more tired than you feel. The good news: frown lines respond well to medical treatment. Which method makes sense depends on whether the line only appears when you frown or is already etched into the skin at rest. In this article, written from our practice in Glattpark (Opfikon) near Zurich, we explain what helps, how long it lasts and what it costs.

In short: how can frown lines be treated?

In most cases the most effective treatment for frown lines is a physician-performed, muscle-relaxing wrinkle treatment. A tiny amount of an active substance is placed precisely into the muscles between the eyebrows. These muscles then pull less strongly, the skin smooths out and – with careful dosing – your expression stays natural. If the line is already deeply etched, a very small amount of soft hyaluronic acid can be added with great caution.

Creams, serums and facial exercises can support skin quality but cannot remove a pronounced frown line. An honest goal is a clear softening, not a frozen, “ironed” forehead.

  • Mild, dynamic frown lines: muscle-relaxing wrinkle treatment
  • Deep, static frown lines: wrinkle treatment, possibly combined with a small amount of hyaluronic acid
  • Alongside: daily sun protection and good skin care to slow progression

Why do frown lines develop?

Between the eyebrows sits a small but strong group of muscles. They pull the brows together and downwards – when you concentrate, squint into bright light or feel annoyed. Repeated over decades, this movement leaves its mark on the skin.

The muscles of the glabella

The main players are the corrugator supercilii, which draws the brows towards the centre and creates the vertical lines, and the procerus at the root of the nose, which tends to cause horizontal lines. Together with other muscle fibres they form the so-called glabellar complex. Muscle strength and position vary greatly from person to person – which is why there is no standard dose that suits everyone.

Dynamic versus static lines

As long as the line only appears when you frown, it is a dynamic line. With age, skin loses collagen and elasticity, and the line remains visible even at rest – it becomes static. UV exposure, smoking and genetics speed this up. The earlier strong muscle activity is softened, the less deeply the line becomes etched.

Which treatment suits my frown lines?

We never decide based on a photo but after a personal examination. We assess the line at rest and in motion, the position of the eyebrows, skin thickness and facial symmetry.

Before every treatment we analyse muscle activity and mark the injection points individually.

Muscle-relaxing wrinkle treatment

For typical frown lines, aesthetic wrinkle treatment comes first. Using a very fine needle, the active substance is placed at a few precisely defined points in the glabellar complex. Muscle activity is reduced but, if you prefer, not switched off completely. You will find all zones and prices on our page wrinkle treatment in Zurich (in German).

Hyaluronic acid for deep, etched lines

If a fine groove remains after the muscles have relaxed, it can be filled with a small amount of soft hyaluronic acid. However, the area between the eyebrows is considered anatomically delicate because blood vessels run here that are connected to the blood supply of the eye. We therefore use filler in this area only with a clear indication, in minimal amounts and with great care.

What skin care and lifestyle can do

Consistent UV protection, good sunglasses against glare and skin care suited to your skin slow down wrinkle formation. They do not replace treatment of a visible frown line, but they help results last longer.

The procedure at our practice in Glattpark

Frown line treatment is a short outpatient procedure. Most patients go straight back to work or home afterwards.

  • Free medical consultation with analysis of your facial expressions and goals
  • Information on effects, alternatives, risks and costs
  • Cleansing and disinfection of the skin, marking of the injection points
  • Treatment with a very fine needle, usually 10 to 15 minutes
  • Free follow-up after 2 to 3 weeks, with fine adjustment if needed

When does it work and how long does it last?

The effect is not immediate. First changes are usually noticeable after three to five days, the full result after about two weeks. Plan treatment well ahead of important events.

Results typically last around four to six months; with very active facial expressions or a fast metabolism, they may fade sooner. Muscle activity then returns gradually – there is no “rebound”, so the line does not become deeper than before. Many patients find that, with regular treatment, their frown lines become less etched over the years.

Risks, side effects and when we advise against it

In experienced medical hands, frown line treatment is well tolerated. As with any medical procedure, there are possible side effects, which we discuss openly beforehand.

  • Common and harmless: slight redness, small bumps or bruises at the injection sites
  • Occasional: a mild headache on the day of treatment
  • Rare: temporary drooping of the upper eyelid or uneven brows – usually resolves within weeks
  • No treatment during pregnancy or breastfeeding, with infections in the treatment area or certain neuromuscular disorders

Aftercare

Please do not massage or rub the treated area for the first few hours, avoid lying flat for about four hours and skip sport, sauna and sunbeds on the day of treatment. Otherwise, your daily routine is hardly affected.

Frequently asked questions

Can frown lines be removed permanently?
Permanent removal is not possible with non-surgical methods. Muscle-relaxing wrinkle treatment lasts around four to six months; with regular repeat treatments, however, the line often becomes less etched over the years.
Is it better to fill frown lines or relax the muscles?
For most frown lines, muscle relaxation is the first choice because it treats the cause – the muscle pull. Hyaluronic acid is only used as an add-on for deeply etched lines and in very small amounts, as the area is anatomically delicate.
Does frown line treatment hurt?
Most patients describe the fine needle pricks as brief and very tolerable. The treatment takes only about 10 to 15 minutes; anaesthesia is usually not needed.
Will I still look natural afterwards?
Yes, when dosing is individual and conservative. Our aim is a relaxed, fresh look in which you can still move your brows. Fine adjustments can be made at the follow-up after 2 to 3 weeks.
